Death Registration and Death Certificate

An application to register a death and obtain a copy of the death certificate.

What it is

A death must be registered within 21 days; after that a separate late-registration procedure applies. The certificate is the official copy of the entry.

What it is used for

  • Heir entry
  • Bank accounts and deposits
  • Insurance claims and pension
  • Changes in land or house records

Documents usually needed (guidance)

Please confirm the final list of documents at your Gram Panchayat office. This list is general guidance, not an official list of legal conditions.

  • The hospital's death report (if any)
  • A copy of the deceased person's identity card (if available)
  • A copy of the applicant's identity card
  • A copy of address proof
  • Additional documents the Registrar asks for (if registering late)

How to apply

  1. Register the death with the birth-and-death Registrar at the Gram Panchayat within 21 days.
  2. Fill in the details exactly as in the documents.
  3. Attach copies of the available documents.
  4. After submitting, take an acknowledgement or inward number and keep a copy of the application.
  5. After receiving the certificate, check the name, date and place.

Where to apply

The birth-and-death Registrar at the Gram Panchayat (usually the Gramsevak / Gram Panchayat Officer)

Fee and time

Any fee is fixed by the Government or by law and is taken against an official receipt. No amount or time limit is stated here. If the service is notified, confirm its time limit on the Aaple Sarkar portal or at the Gram Panchayat office.

Please note

The heir entry for land is made by the revenue department (Talathi office); changing the name on the house-tax record is a separate application to the Gram Panchayat. These entries are separate from each other.
